Operation Armor of God Liability Waiver: Due to the outbreak of the novel Coronavirus (COVID-19), Church of the Living Christ is doing everything we can to protect you, your children, our community, and our staff and volunteers. To this extent, Church of the Living Christ will be following the Center of Disease Control (CDC) with regard to social distancing practices and sanitation. Symptoms of COVID-19 include:FeverFatigueDry CoughDifficulty BreathingI agree to the following:My child, nor members of my household, will attend Church of the Living Christ/Operation Armor of God Vacation Bible School if they have experienced any of the symptoms listed above.My child, nor members of my household, will not attend Church of the Living Christ/Operation Armor of God Vacation Bible School if they/we have traveled internationally.My child, nor members of my household, will not attend Church of the Living Christ/Operation Armor of God Vacation Bible School if they/we have traveled to a highly impacted area within the United States of America.My child, nor members of my household, do not believe that we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus (COVID-19).My child, nor members of my household, have not been diagnosed with the Coronavirus (COVID-19).Church of the Living Christ/Operation Armor of God Vacation Bible School cannot be held liable for any exposure to the Coronavirus (COVID-19).My child has permission to participate in all scheduled activities with Church of the Living Christ/Operation Armor of God Vacation Bible School and any other scheduled activities customarily associated with the Operation Armor of God event. My child is physically fit and adequately prepared to participate in all recreational and outdoor activities.I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that my child is injured or becomes ill. I understand that Church of the Living Christ/Operation Armor of God Vacation Bible School will not be responsible for medical expenses incurred solely on the basis of this authorization.The leaders and designated adult chaperones reserve the right to restrict my child from any activity that they do not feel is within the physical capabilities of my child.By signing and dating the box below, I understand and agree to the conditions stated in this waiver.
