The Castle of Courage Liability Waiver:

Although the COVID-19 pandemic seems to be waning, the Church of the Living Christ is doing everything we can to protect you, your children, our community, and our staff and volunteers. To this extent, the Church of the Living Christ will be following the Centers for Disease Control and Prevention (CDC) guidelines. 

Symptoms of COVID-19 include:
Fever
Fatigue
Dry Cough
Difficulty Breathing

I agree to the following:

My child, nor members of my household, will attend the Church of the Living Christ/”The Castle of Courage” Vacation Bible School (VBS) if they have experienced any of the symptoms listed above.

My child, nor members of my household, will not attend the Church of the Living Christ/”The Castle of Courage” VBS if they/we have traveled internationally.

My child, nor members of my household, will not attend the Church of the Living Christ/”The Castle of Courage” VBS if they/we have traveled to a highly impacted area within the United States of America.

My child, nor members of my household, do not believe that we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus (COVID-19).

My child, nor members of my household, have not been diagnosed with the Coronavirus (COVID-19).

Church of the Living Christ/”The Castle of Courage” VBS cannot be held liable for any exposure to the Coronavirus (COVID-19).

My child has permission to participate in all scheduled activities with the Church of the Living Christ/”The Castle of Courage” VBS and any other scheduled activities customarily associated with “The Castle of Courage” VBS.

My child is physically fit and adequately prepared to participate in all recreational and outdoor activities.

I understand that I will be notified in the case of a medical emergency. However, in the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event that my child is injured or becomes ill. I understand that the Church of the Living Christ/”The Castle of Courage” VBS will not be responsible for medical expenses incurred solely on the basis of this authorization.

The leaders and designated adult chaperones reserve the right to restrict my child from any activity that they do not feel is within the physical capabilities of my child.

By signing and dating the Registration Form, I understand and agree to the conditions stated in this waiver. I also give consent for my child to participate in the events of “The Castle of Courage” VBS and release the Church of the Living Christ from any liability.
